August 2026 Letter to Parents of Freshman and Transfers
Required forms ALL Freshman and Transfers
Physical Form and Confidential Health Record Packet: All incoming freshmen and transfers are required to submit the following health records to the Main Office by September 11, 2026.
Additional if Applicable
Diabetic Orders and Plan - If your child is a diabetic, please have this form be completed by the child's physician and returned to the Nurse's Office.
Epi-Pen and Inhaler Orders - If your child is at risk of anaphylaxis, please complete this form and return it to the Nurse's Office.
Asthma Treatment Plan - If your child has Asthma, please complete this form and return it to the Nurse's Office.
Individual Emergency Health Plan - If your child needs specialized care and/or medication in the event of a medical emergency, please complete this form and return it to the Nurse's Office.
Required Athletes
STA Athletics Physical Form - If your student is an athlete, a yearly physical is required to be completed prior to the competitive season. Please ensure all documents are completed and uploaded to our ArbiterSports application.
