STA Physical Form - A complete physical is required of all freshmen and juniors. If your student is an athlete, a yearly physical is required to be completed prior to the competitive season.
STA Sudden Cardiac Death Packet - All students and their parent/guardians are required to adhere to and acknowledge state regulations regarding sudden cardiac death.
Diabetic Orders and Plan - If your child is a diabetic, please have this form be completed by the child's physician and returned to the Nurse's Office.
Epi-Pen and Inhaler Orders - If your child is at risk of anaphylaxis, please complete this form and return it to the Nurse's Office.
Asthma Treatment Plan - If your child has Asthma, please complete this form and return it to the Nurse's Office.
Individual Emergency Health Plan - If your child needs specialized care and/or medication in the event of a medical emergency, please complete this form and return it to the Nurse's Office.
