Medical Records Required for All New Students

Prior to entering school, no later than August 1 and no earlier than June 1, all incoming freshmen and transfer students must have a current physical.  THIS IS A BOARD OF HEALTH MANDATE.   The school issued Physical Examination Form and the Health History Form (athletic and non-athletic) and a copy of immunization records must be returned to St. Thomas Aquinas High School, Attention:  School Nurse.   Each of the forms must be signed by the parent, student, and physician as indicated on the forms. 

If the student requires the use of an Epi-pen, inhaler or any other medication during school hours, including over-the-counter remedies, i.e., Tylenol, ibuprofen, Sudafed, cough syrup, etc, please have his/her physician complete the Self-Carry and IHP Form and Medication Form, no later than August 1.   These forms are attached, available on the website at www.stahs.net (under student life), and are available in the Nurses’ Office.   

Any questions, related to the completion of these forms, can be directed to the Nurses’ Office, 732-549-1108, extension 642 or 619.  It is important to note, the attached forms have been approved by the State of New Jersey; therefore, you must use these specific St. Thomas Aquinas High School Forms.  It is recommended that you keep a copy of all medical related forms for your records.
