Winter Registration Now Open and Due November 1

St. Thomas Aquinas High School has partnered with ArbiterSports for all of our athletic forms and managment needs. Every STA athlete's parent/guardian must obtain a completed Preparticipation Physical Evaluation Form (from doctor) and complete the ArbiterSports registration by the deadlines indicated below. If an athlete has previously competed in the Fall of 2025, the Health History Update Questionnaire must be completed and uploaded to update the winter registration below by November 1.

Required Forms:

Preparticipation Physical Evaluation Form - Required at the start of the first season of particpation. This must be printed, completed, and signed by a parent/guardian and uploaded to that season's Arbiter registration. 

Health History Update Questionnaire - Required for each subsequent season an athlete particpates in. This must be printed, completed, and signed by a parent/guardian and uploaded to that season's Arbiter registration.
